COURSE OVERVIEW

Unfortunate incidents can have detrimental effects on your organization, such as: decreased morale, tarnished image, legal expenses, productivity loss, disqualification from contract bids, and it could potentially drive you out of the market.

This Web-based Incident Analysis program is often referred to as Incident or Accident Investigation. We prefer to call this program Incident Analysis for two reasons; the term investigation may imply an interrogation seeking to point fingers, and secondly; we are indeed carrying out an analysis of an incident to introduce corrective measures to prevent its recurrence.

The aim of an incident analysis is prevention. Identifying the causes of the incident and implementing measures to manage or eradicate those causes can prevent similar incidents in the future. Incidents are seldom due to a single cause; factors such as work environment, job constraints, supervisory or worker experience can all contribute. These factors need to be investigated to ascertain their respective roles in the incident.

Once the causes are identified, corrective measures need to be formulated and executed to prevent recurrence. The analysis team should always remember that effective incident analysis involves fact-finding, not finger-pointing.

For an analysis to be a useful tool, three steps must be followed:

  1. Diligent collection and analysis of information,
  2. Execution of corrective actions, and
  3. Monitoring of those actions to evaluate effectiveness.
