It’s commonly said that most accidents are preventable, but senior safety inspector once told us that they aren’t just preventable, but they are eminently preventable. He explained that in more than 80% of the incidents that he investigated, the signs were all there, people could see them and if they really looked, it would be possible to prevent. We want to explore this thinking and share ideas with community.
