Instead of waiting for someone to learn a lesson the difficult way, our goal is to collect insights from seasoned safety leaders and pass them along. Many professionals believe that most incidents are avoidable, but one senior inspector once told us something even more striking: incidents aren’t just avoidable, they are obviously avoidable.
He explained that in more than 80% of the cases he reviewed, indicators were present long before an incident occurred. Hazards were visible, warning signs were clear, and with true attention, the outcome could have been changed.
