An incident, in the broadest sense, refers to an unexpected event. Timely reporting and investigating of such incidents is the quickest way to bring about OH&S improvements. Therefore, incident investigation plays a critical role in every health and safety management system.

COURSE OVERVIEW

Incident investigation is more than just completing a report form. It involves the collection of information and the drawing of conclusions. The report form is merely a means to document this process.

The Incident Investigation VR 6.0 course doesn’t aim to attribute fault or assign blame. Its dual objectives are to:

  • identify the causes of the incident, and
  • implement corrective actions to prevent recurrence.

This Incident Investigation VR 6.0 course provides 2.5 technical hours.

BCRSP
This course may be eligible for 0.25 CEUs for the Board of Canadian Registered Safety Professionals (BCRSP) – Continuing Professional Development Program (CPD Program) for the purpose of recertification in maintaining the BCRSP certification. COURSE TOPICS


Upon completion of the Incident Investigation VR 6.0 course, participants will have the ability to:

  • outline the objectives and advantages of incident investigations, 
  • detail how to prepare for and carry out an investigation,
  • identify and differentiate between immediate (direct) causes and underlying (root) causes of incidents, and
  • formulate recommendations for
    remedial actions to stop incidents from happening again. The course includes
    classroom instruction, group discussions, exercises, and an exam.

Estimated 2 ½ hours

PASS MARK

The testing included in this online course is intended to reinforce the information provided. A minimum score of 80% is required to pass this course.

CERTIFICATE OF COMPLETION

Upon successful completion of this online course, a certificate of completion will be available for download and printing. This certificate is valid for 3 years from the date of completion printed on the certificate unless otherwise specified by an employer, legislation, or other authority. This course is recommended to be retaken every 3 years from the course completion date indicated on the certificate.
