Every job site runs on energy (such as electrical, hydraulic, thermal, etc.). But when that energy is uncontrolled, it becomes a serious threat. Lockout/tagout (LOTO) procedures exist to keep that power in check, protecting workers from some of the most severe injuries imaginable: amputations, burns, electrocution, and crushing incidents.
Thousands of workers are injured each year due to the unexpected start-up of equipment or the release of stored energy. The tragedy? Nearly all of them could have been prevented. Lockout/tagout isn’t just a regulatory checkbox, it’s an act of protection.
Seeing Energy for What It Really Is
LOTO starts with one essential skill: the ability to recognize how energy behaves within a system. Machinery rarely has a single power source, it’s often a mix of electrical circuits, pressurized systems, or suspended weights.
Before repairs begin, each power source must be:
- Identified
- Isolated
- Neutralized
Primary energy sources are obvious. These are usually electrical panels, fuel lines, or thermal systems. But stored or residual energy is the trickier adversary (such as compressed air, hydraulic pressure, tensioned springs, or even gravity acting on elevated loads). These forces can release violently if not dissipated or restrained.
The takeaway for safety leaders? Teach workers to think like investigators. Every valve, capacitor, or moving part should be evaluated for hidden energy that could surprise them.
Roles That Anchor Responsibility
Effective lockout/tagout programs have clearly defined roles.
- Authorized employees apply locks and tags, control energy, and verify isolation. They must be trained to identify every potential energy source and apply LOTO devices correctly.
- Affected employees work near equipment being serviced. Their role is to recognize tags and never attempt to restart machinery under LOTO.
- Other employees may work in the area but are still part of the communication chain, each one must interpret LOTO tags as absolute stop signs, not suggestions.
Employers also carry weighty obligations: providing equipment, maintaining documentation, conducting audits, and critically, ensuring every worker’s competency through training and retraining.
Building LOTO Procedures That Actually Work
Each piece of machinery needs its own, machine-specific LOTO plan, ideally written on paper. That means identifying every energy source, isolating points, and defining a precise sequence for de-energization and verification.
A well-built procedure enables workers to evaluate the system they’re locking out, anticipate stored energy, and apply controls that match the hazard. Every lock, tag, and test must be traceable to a specific person. Vague assumptions are where LOTO programs often fail; “everyone knows how to do it” is not a procedure, “follow the written sequence until you verify isolation” is.
The Nine Steps That Save Lives
Whether you’re in a manufacturing plant, a trucking repair bay, or an oil sands site, the nine foundational steps of lockout/tagout never change.
- Prepare for shutdown: Identify the equipment and its energy sources.
- Notify affected employees: Communicate before acting.
- Shut down and deactivate the system: Bring it to a zero-energy state.
- Isolate the system from hazardous energy: Physically disconnect or block it.
- Dissipate or restrain stored energy: Bleed, vent, or block moving components.
- Attach locks and tags: One lock, one key, one person every time.
- Release any remaining energy: Verify no residual hazards remain.
- Perform maintenance or service: Safely complete the task or wait for a trained professional to do so.
- Release from lockout: Inspect, communicate, and restore power only when every worker is clear.
Note that each step reinforces the next. This is a process of deliberate verification designed to prevent tragedy.
Isolation, Verification, and the Discipline of Safety
Before any service begins, the system must be tested to prove it won’t start; workers should attempt to operate controls to confirm that all energy sources are indeed neutralized. This single step, verification has prevented countless injuries.
In group settings, lock boxes or group lockout systems help coordinate multiple teams without sacrificing accountability. Whether it’s a mill shutdown or refinery turnaround, each worker owns their lock, their tag, and their safety.

Behind every procedure is a person who has to make decisions under pressure. Fatigue, familiarity, and production demands often test even the best the most safety-conscious workers. Supervisors can reinforce this mindset by conducting safety audits and encouraging crews to challenge unsafe habits before they become incidents. Over time, consistency builds confidence and confidence saves lives.
