It involves the collection of information and the drawing of conclusions. The report form is merely a means to document this process. <\/p>\n<p>The Incident Investigation VR 6.0 course doesn&#8217;t aim to attribute fault or assign blame. Its dual objectives are to: <\/p>\n<ul>\n<li>identify the causes of the incident, and <\/li>\n<li>implement corrective actions to prevent recurrence.<\/li>\n<\/ul>\n<p><\/p>\n<p>   This Incident Investigation VR 6.0 course provides 2.5 technical hours.<\/p>\n<p><b>CEUs<br \/><\/b>(Continuing Education Units) <\/p>\n<p>BCRSP <br \/>This course may be eligible for 0.25 CEUs for the Board of Canadian Registered Safety Professionals (BCRSP) \u2013 Continuing Professional Development Program (CPD Program) for the purpose of recertification in maintaining the BCRSP certification. See the BCRSP website at <a href=\"http:\/\/www.bcrsp.ca\">www.bcrsp.ca<\/a> for CPD point criteria.  <\/p>\n<p><\/p>\n<div class=\"title-bar\">   <span style=\"font-size: 18.0px;float: none\">     <b>COURSE TOPICS<\/b><\/span>   <\/div>\n<p><span><br \/><\/span><\/p>\n<p><span>Upon completion of the Incident Investigation VR 6.0 course, participants\u00a0<\/span><span>will have the ability to:<\/span><\/p>\n<ul>\n<li>outline the objectives and advantages of incident investigations,\u00a0<\/li>\n<li>detail how to prepare for and carry out an investigation,<\/li>\n<li>identify and differentiate between immediate (direct) causes and underlying (root) causes of incidents, and <\/li>\n<li>formulate recommendations for<br \/>remedial actions to stop incidents from happening again. The course includes<br \/>classroom instruction, group discussions, exercises, and an exam.<\/li>\n<\/ul>\n<p><\/p>\n<div class=\"title-bar\">   <span style=\"font-size: 18.0px;float: none\">     <b>ONLINE COURSE DURATION<\/b><\/span><\/div>\n<p><\/p>\n<p>Estimated 2 \u00bd hours<\/p>\n<p><\/p>\n<div class=\"title-bar\">   <span style=\"font-size: 18.0px;float: none\">     <b>PASS MARK<\/b><\/span><\/div>\n<p><\/p>\n<p>The testing included in this online course is intended to reinforce the information provided. A minimum score of 80% is required to pass this course.<\/p>\n<p><\/p>\n<div class=\"title-bar\">   <span style=\"font-size: 18.0px;float: none\">     <b>CERTIFICATE OF COMPLETION<\/b><\/span>   <\/div>\n<p><\/p>\n<p>Upon successful completion of this online course, a certificate of   completion will be available for download and printing.
